What it is

SimpleSurvey is an online survey and forms platform that calls itself "Canada's platform for secure surveys, data collection, and online voting". It is built for organizations rather than hobbyists: the homepage says it is trusted by public agencies, healthcare institutions, professional associations, consulting firms and businesses across Canada. The footer reads "© 2007 - 2026 | Proudly Canadian", and the contact details list a Montreal number. The selling point is that your data stays in Canada.

What you build and how

You build surveys, forms, quizzes, polls and votes. The builder lets you start from scratch, from a copy of an earlier project or from a saved template, and add a title, subtitle, sections, subsections and HTML text blocks. The plan table says the surveys you create can be accessible to WCAG 2.1 AAA, which matters for public bodies. The features page says you choose from a variety of question types and use logic, branching, quotas, API integrations and workflow triggers to build and launch even complex projects.

The examples page shows the range: satisfaction surveys, course evaluations, dynamic quizzes, exit surveys, tailored sign-up forms, software feedback, embedded polls that display real-time results on your website, community attitude surveys and employee engagement surveys. The company names government, health, education and nonprofit sectors. Because the platform also advertises online voting, and its professional services cover "live votes, and elections", it reaches past ordinary feedback forms into governance use.

Logic and automation are where the product gets more advanced. Quotas are a built-in feature. A communication workflow feature automatically triggers tasks and dispatches information based on survey responses and conditions, so that, in the company's words, responses become actionable items you can respond to immediately. The contact collector generates contact files from responses, assigns them to groups and follows up with later surveys, and the company calls it great for registration forms, events, patient intake and long-term studies. A secure access page makes respondents enter credentials through a login page before they can reach a survey.

Distribution works through email invitations, embedded polls on your site, and optional SMS. The plan table sells SMS blocks as an add-on for sending invitations and reminders by text. A Web API connects SimpleSurvey with your existing systems to automate workflows, manage survey data and exchange information between platforms. The plan table marks API with an asterisk, so check which tiers include it.

Branding and sharing are handled by white labeling, which lets you use your own domain and drop every reference to SimpleSurvey, including in the project URL and in email invitations. An end-users feature grants internal or external stakeholders access to survey dashboards and reports. Reporting is described as real-time, with live reports and analytics, so you can watch results build.

Teams collaborate on Team and Enterprise tiers, where users share surveys, templates, trusted logos and other assets, which the company says reduces project creation time. The plan table says support comes in English and French inside the app, with a bilingual user guide. Priority support is a line item in the plan table, and it means the support request handler gives your tickets priority and, if email doesn't resolve an issue quickly, staff call you.

Beyond software, SimpleSurvey sells professional services: consultants help create surveys, forms, live votes and elections, run data analysis, onboard your team and deliver turnkey projects.

The evidence from the pages

The homepage and plan page list 4 tiers. SOHO is $31 a month billed $372 a year. PRO is $58 per user per month billed $696 a year. TEAM is $96 per user per month billed $1,152 a year. ENTERPRISE has no listed price and is quoted on contact. The plan page also shows a shorter commitment: billed every 3 months, SOHO is $49.67 a month ($149 per quarter), PRO is $83 per user per month ($249 per quarter) and TEAM is $141.67 per user per month ($425 per quarter). Choosing yearly billing saves the equivalent of 3 months.

Usage limits are in the plan table: projects, and responses that are prorated to the subscription period and include partial responses. Limits can be raised at any time at additional cost, and the table lists 10,000, 15,000 and 20,000 response steps next to a contact option. The FAQ says plans are priced by included features, number of users, usage limits and add-ons. The Enterprise page pitches "a centralised data collection tool at a fraction of the cost of others on the market", a comparison claim it makes without figures. The FAQ says you can sign up for a free trial or contact the company to discuss the right plan, and it points to a Master Subscription Agreement for the full terms of use. The trial is stated in 2 ways, a free trial with no credit card required on the homepage and a free 15-day trial on the examples page.

On security and hosting, the homepage says your data lives on Microsoft Azure in Canada, with access controls and audit logs built in. The company wording on compliance is careful: the platform "provides tools and controls to help clients align with applicable federal and provincial laws, including PIPEDA". The Enterprise page says servers are located only on Canadian soil, always, and that Canadian data residency is the default. The FAQ adds that hosting is meant to meet Canadian data sovereignty, residency and privacy requirements. The company says it has served Canadians "at home and abroad for over a decade".

Who it suits and who it doesn't

SimpleSurvey suits Canadian public agencies, hospitals, associations and consultancies that must show where survey data lives, and that want accessible, bilingual surveys, audit logs and optional white labeling. A professional association running an online vote, a health organization doing patient intake or an institution running course evaluations are the kinds of projects the pages describe. Small teams can start on SOHO, and larger groups get collaboration, shared assets and end-user dashboards on Team and Enterprise.

Limitations are worth knowing. Hosting is Canada only, which is a feature for some buyers and a constraint for a company that needs data in another region. PRO and TEAM are priced per user, so costs rise with headcount. This is a survey and data-collection platform, not an app builder, so its automation stays inside the survey lifecycle. So check the API and workflow options against your own systems before committing.
