What MakeForms is

MakeForms is a no-code form creator and automation tool. Its homepage title calls it an "All-in-One No-Code Form Creator & Automation Tool", and the headline pitches an "Enterprise ready form builder made for fast-moving teams" that offers "the speed of a modern form builder with the security and control serious businesses expect." It is aimed at operations teams, lead-generation and sales teams, and organizations with compliance needs such as healthcare providers and NGOs.

The copyright line names Creative Tools Inc as the company behind it. The homepage announces 2 new apps beside the form builder, MakeForms Scheduling and MakeForms Signing, so the product is growing from forms into adjacent booking and document work.

What you build and how

Forms. The product menu lists a Form Builder, Step Forms, OTP Verification Forms, PDF Forms, HIPAA Forms, Lead Generation forms, Workflows and Salesforce forms. The feature menu adds Conditional Logic, Collaboration, KYC Forms, Form Embeds, Data Residency, Notifications, a Multi Step Form Builder and Document Workflow Automation. The company describes the builder as one for "sleek, customizable forms" that capture more responses and plug into your workflow, from simple surveys to advanced logic-driven forms.

Form experiences. The comparison page lists 3 formats: conversational forms that show one question at a time, step forms across several pages, and all-at-once forms. Its feature table also ticks multi-language forms, calculator and calculated fields, payment collection, custom domain hosting, removal of platform branding on all paid plans, and an unusual option to fill a form inside an email through AMP.

AI. An AI form builder sits at the top of the menu with a "New" tag: you describe your form and the AI scaffolds the structure in seconds. The homepage links to a post introducing "Vibe Coding for Forms" and a MakeForms AI Co-Pilot. The AI tools menu also names an AI Quiz Builder, an AI Multistep Form Builder, an AI Multilingual Form tool, PDF To Form and Document To Forms, which turn existing files into forms.

Conditional logic. This feature has its own page, and it is the most detailed description of how the builder behaves. The company calls it a visual logic builder: "No code. Just select the rules and set the conditions." The rules it lists:

  • Show or hide fields depending on what the respondent enters.
  • Skip full pages in a multi-step form with a Skip Step action.
  • Copy a value from one field into another with a Change Value action, so people are not asked twice. Its example pre-fills a country code field with "+91" when the respondent selects India.
  • Disable submissions for ineligible people, for gated flows and eligibility checks.
  • Redirect to a page or URL based on the answers.
  • Change the email notification recipient, with the example of notifying the IT team instead of the general inbox when the form is submitted for "Technical Support".
  • Route responses to integrations. The page names CRM, Slack and webhook as examples.

The comparison table marks conditional logic as available on all paid plans.

Verification. MakeForms includes verified submissions where respondents confirm their identity with one-time codes before submitting, which is the OTP Verification Forms product. A homepage review from an event organizer says fake registrations were a problem for capacity-limited events and that MakeForms "resolves this problem through SMS verification."

Documents and signatures. Document and PDF generation and e-signatures are listed from the Pro plan, and the comparison page says Pro "combines forms, PDF generation, and e-signatures" so teams avoid stitching extra tools together. A homepage blog post covers automating document approval workflows, and the separate Signing and Scheduling apps extend the same idea.

Integrations and workflows. The feature table names Salesforce, Google Sheets, Zapier and Make, payment collection, and Keragon, described as HIPAA healthcare automation. The conditional logic page adds routing to a CRM, Slack or a webhook, and the footer links a full Integrations page. Another homepage post is about turning every form into a lead capture engine.

Team and publishing. Multi-user team access with roles is listed from Pro. Forms publish to your own domain or embed wherever needed; one review on the homepage says the custom domain setup "was a breeze", and the comparison page says MakeForms embeds well on Wix, though not as a native Wix app.

What the pages say about plans and compliance

Pricing. The comparison page states "MakeForms Pro: $69/month" and says it includes HIPAA, SOC 2, team access and no feature gates, and elsewhere that Pro brings a HIPAA BAA, SOC 2 Type II, team access, document automation and e-signatures. It says the free plan has unlimited forms with no submission cap and no backlink required, and that all paid plans have unlimited submissions. An Agency plan is named alongside Pro: "Yearly Pro and Agency plans include Concierge so our team can build forms inside your account," and the page says Concierge migrates your forms if needed. Signup is "Free plan, no credit card. Ready in under a minute." The homepage closes with "No CC required" and offers both a free start and a personalized demo. Pro is the only plan these pages price.

Compliance and data location. This is the center of the pitch. The company says Pro comes with a HIPAA BAA, self-serve and without a sales call, and that MakeForms is "SOC 2 Type II certified" on all plans; its feature table also ticks ISO 27001 and GDPR compliance. The homepage claims MakeForms "is the only form builder that lets you decide in which country will you save your forms' data." It lists data regions in the United States (HIPAA compliance), the European Union (GDPR compliance), Canada (PIPEDA compliance), the United Arab Emirates, Israel, South Africa, India, Australia, Japan, the United Kingdom, Brazil and Singapore, with custom data regions for Enterprise customers. The comparison page puts Canadian, Australian and Singapore residency from Pro, and the footer has landing pages for EU hosted, Australia and Canada form builders. These are the company's own statements.

Social proof. The homepage shows G2 Reviews at "4.9 out of 5". NorthernStar Milk is quoted saying "MakeForms has the Canadian data storage and the PIPEDA compliance that really sold us on the switch." An operations lead at a B2B SaaS team writes "We replaced multiple fragmented tools with MakeForms and immediately felt the product maturity." Sahil Khosla calls it a "Mercedes" of forms after a demo. One reviewer needed a PIPEDA form maker "that could handle single forms for multiple clinic locations" and says the development requests they made were quick; another says the team added a suggested feature "just 2 days later". Help comes through Get Help, Docs and a public Roadmap, and the company runs an affiliate partner program.

Who it suits, and the caveats

A healthcare practice, an NGO or a financial team that needs a HIPAA BAA or SOC 2 Type II on a self-serve plan has a clear reason to look. So does a team with a country-specific storage requirement, since residency is built into the offering across a long list of regions. An operations team that needs branching forms, OTP checks, conditional notifications, document generation and Salesforce routing without code gets those on the feature list. An agency can use custom domains, embeds and the Agency plan's Concierge service for client forms, and a lead-generation team gets conversational and step formats.

The caveats are modest. Conditional logic, branding removal and the compliance features sit on paid plans, so a free-plan team gets unlimited forms but a narrower toolset. And the product embeds on Wix rather than running as a native Wix app, which matters to a shop built entirely on Wix.
