What Formplus is
Formplus is an online form and survey builder, sold on the idea of a paperless office. The home page is titled "Create Free Online Forms & Surveys in 2 Mins" and invites you to "Switch to a paperless office with functional online and offline (beta) forms." It is built for teams that collect applications, registrations, payments, orders, bookings and survey responses without a developer. Its solutions menu groups the product by job: online surveys, data collection, registration forms, application forms, e-signature, booking and reservations, payment forms, order forms, offline forms, contact forms and sign-up forms, and the footer adds a quiz maker.
The reader here is mostly the operations or administration lead: a school office, a small business, a membership organization or a nonprofit that needs structured intake, payment collection or field data gathering. The customer stories on the home page come from exactly that mix, from a treehouse lodge to a ballet academy to a charter school.
What you build and how
Forms are assembled in a drag-and-drop builder. The features page lists over 30 form fields, field validation, calculating form fields, file upload, multi-page forms, form logic, lookup fields and digital signatures. The home page counts "20+ field types" in one place, while the features page and the pricing comparison say 30+. Beyond the basic inputs, the company names date-time, digital signature and calculation fields as its advanced options, and says calculation fields handle simple math such as scoring. Templates are a large part of the pitch: the home page mentions "hundreds of form templates," and the pricing comparison page says "more than 1,000 templates" across types, use cases and industries, including consent forms and authorizations, each added to the builder with a single button.
Logic has its own feature page. Conditional logic lets you "Show or hide form fields based on user responses," and the page says it applies to whole pages as well as fields, giving "short forms with better conversion rate" because respondents only see what applies to them. A customer at Gensol says the "dynamic questions based on another question feature helped us" in half-yearly employee performance appraisals, together with the Google Sheet sync it used for analysis and individual reports.
The form options go well past a single page of questions. The features page lists offline forms, save and resume, intro and post-submission pages, location-enabled forms, user redirect and customization. Save and resume lets a respondent store a partly completed form and return later, and the pricing page also lists partial form submissions and custom domains. Offline mode is the feature customers single out. The company says people can fill in and submit a form without internet access, and the submission uploads to the Formplus database as soon as the connection returns. A PactWorld data and program support lead says it mattered most because its teams work in communities with little or no internet access. The home page still labels offline forms as a beta.
Signatures are handled natively. You drag the signature field into the form, and signed submissions are saved as images in your preferred external storage or on Formplus servers for download.
Document output is the newest piece. The banner on the home page announces Form2Doc: "generate polished, branded documents from every form submission, automatically." The features page lists document merge, the pricing table lists form notifications in Word or PDF formats, and a separate PDF Builder creates downloadable PDF documents "such as legal contracts, quizzes, questionnaires, forms and many more." A membership and data manager at AASHE says Form2Doc lets the organization send certificates of participation and agreements that people sign through the form.
Payments run through the form. Formplus lists Stripe, Flutterwave, Paystack and PayPal as supported gateways and says you can "receive payments from more countries right in your form." A testimonial from the Pennsylvania Academy of Ballet describes customers registering and paying for classes online, completely contactless.
Integrations are wide for a form tool. The home page says forms connect to Google Sheets, Google Drive, Microsoft OneDrive and Dropbox "with just a click," and its integration strip also shows Trello, Slack, Salesforce, Airtable, Mailchimp, Discord, Asana, FreshDesk, WordPress, Basecamp, SMS by Zapier, Google Contacts, Breezy HR, MySQL, Active Campaign, Google Calendar, Gmail, Twilio, Twitter, Acuity Scheduling, Zendesk, webhooks, Keap, Podio and Google Docs. The features menu adds a Zapier integration and data import and export, and responses can be exported as a CSV file.
Sharing covers embeddable forms, a WordPress form embed, custom and shortened form links, forms prefilled from a URL or filled dynamically, social sharing buttons that generate image cards for every link, and a QR code you can add to a website or print. Notifications include email invitations, confirmation emails, confirmation to other addresses, teammate notifications and periodic notifications. Analytics turns form entries into graphs and charts and shows traffic source and location.
For collaboration, the home page says you can invite your team, assign roles and permissions, and grant editing, viewing or sharing access. The features page lists approvals and workflows, and the pricing table adds audit trails.
Plans, limits and claims
Pricing is published on the comparison page, which lists a free version, "Free Forever," and three paid packages. Prices are per month as a flat plan fee, with the annual-billing rate after it:
- Free, $0: 50 monthly form responses, 1 user, 3 forms a month, 100 MB storage.
- Starter, $25 a month or $20 a month billed $240 annually: 1,000 monthly responses, 1 user, unlimited forms, 20 GB storage.
- Professional, $45 a month or $36 a month billed $432 annually: unlimited responses, 3 users, 1 team, unlimited forms, 50 GB storage.
- Platinum, $85 a month or $68 a month billed $816 annually: unlimited responses, 3 users, 2 teams, unlimited forms, 100 GB storage.
The page says there is no one-time payment option, and the toggle marks yearly billing as 20% off. Signing up needs "no credit cards." The plan table lists features such as unlimited file uploads, conditional logic, confirmation emails, payment integrations, prefilled forms, form calculations, audit trails, white-labeled forms, customized form links or a custom subdomain, a Stripe integration, document merge, approvals and lookup fields, without marking which tier each belongs to.
On security, the menu lists GDPR-compliant forms, PCI compliance, private forms, subdomains and secure forms.
On adoption, the home page shows "1562+ Trusted users," "13877330+ Received responses" and "124413+ Forms created." Virginia of Polk State College Charter Schools says Formplus "decreased our time and cost ten times over" by cutting paper applications and that it is "100% faster to obtain enrollment applications." MCAM NorthWest praises the customer service. These are the vendor's chosen quotes. The resources menu points to case studies, a blog, a support center, an FAQ and an enterprise solutions page.
Who it suits and who it does not
Formplus suits schools, small businesses, nonprofits, membership bodies and field programs that want one tool for forms, surveys, payments and documents, with a free plan to start. A school office taking enrollment applications, a studio taking class registrations, a lodge collecting guest details before arrival, or an HR lead running appraisals can build quickly from a template. Field teams in low-connectivity areas get real value from offline collection, and teams that produce certificates, contracts or letters from submissions get the most from Form2Doc and the PDF Builder.
The free plan's 50 monthly responses and 3 forms a month make it a test drive; a live campaign needs Starter or above. Collaboration is modest, with 1 user on Free and Starter and 3 users on the higher plans, which may not suit a larger ops team. Offline forms are still labeled beta on the home page, so field teams should test them before a deployment. And the product describes no database layer or app-building tools, so it is a forms-and-documents tool rather than a platform for building full internal apps.
