FormDesigner.pro, which its pages call Form Designer, is a web form builder that doubles as an online calculator builder. The home page describes it as "a universal solution with which you can create any online form and customize it to your site and your tasks." It is aimed at site owners and small teams who want feedback forms, order forms, quizzes and booking forms running on a website without a programmer. The home page headline is "Builder of web forms, surveys, quizzes and calculators", with the promise to help you "create web forms of any complexity, without a designer or programmer."

What the product is

Form Designer works from templates. The home page says "Using ready-made templates, you can create a feedback form, order form, online booking form, quiz, survey, test or online calculator in a few clicks." The pitch is speed and breadth: one service that covers simple lead capture, order taking, scoring tests and in-form math, with a long integration list behind it. The site is offered in Ukrainian and English. The home page counters claim 177,473 forms created by clients, 8,164,401 submissions received and 1,065,115 form shows per month, and it displays ratings of 4.9/5 and 4.8/5.

What you build and how

The editor starts with a template. The home page says "Choose a suitable template and create your web form based on it, then edit it to your liking." The templates the site describes are:

  • Feedback form. Lets you "keep your email private and receive messages from visitors to your site with anti-spam protection."
  • Application form. "A simple lead form to collect submissions from the site."
  • Order form. Accepts online orders for products and services and keeps "records and statistics of your orders."
  • Subscription form. Collects customer email addresses and connects to email newsletter services.
  • Voting form. "Keeps track of votes and determines the winner," with protection against cheating.
  • Testing form. For research, segmentation and scoring.
  • Quiz. The company says you can create "selling, entertaining, and other quiz forms," with boolean rules and branching supported.
  • Online calculator. "Do the most complex mathematical calculations directly in the form in real time." A customer says they built a simple bonus-malus coefficient calculator in 10 minutes and posted it on their website.
  • Online record form. A calendar lets visitors book a date and time for a set period.

Placement. A finished form can be opened from a direct link on the company's site or on your own subdomain, placed inside a page on your site, or shown as a pop-up. The pop-up can trigger automatically on a condition you set, open when a visitor clicks any element of your site, or open from an "ear" tab fixed to part of the screen.

Built-in controls. The home page lists features that sit around the form itself. A form can detect a visitor's city, region and country from their IP and restrict access if needed, block submissions by IP address, record the browser, its version and the operating system, require confirmation of an application by email, and give users a personal account where they can view their own submissions.

A note on the calculator page says these are "conditional types of forms that can be combined with each other," so a quiz can feed a calculator and a booking form can sit inside an order. The site's example pages cover quizzes, calculators, feedback, order, voting, testing and online record forms, and it has dedicated pages for an online quiz maker and an online calculator builder.

AI inside the builder. The calculator page describes an AI assistant in the builder, where you write a task description and get a ready-made form. The page says it is "available in your account" and "powered by your plan's AI credits." Separately, the company offers an MCP connection for AI clients. You can "manage forms from Claude, Cursor and other AI clients," and the stated abilities are to "create forms, edit fields and view submissions using natural language." The page names Claude, Cursor, Claude Code and ChatGPT among the clients and says the MCP server exposes "about 100 tools" covering forms, submissions, themes, logic and flow.

Integrations. This is the part of the site with the most detail. Ready-made modules are described for several kinds of tools:

  • Email and SMS services: UniSender, GetResponse, MailChimp, ExpertSender, SendPulse, MailerLite, SendFox and Selzy.
  • CRMs: Bitrix24, Megplan, FreshOffice, Bpium, PrimeGate CRM, S2 CRM (Salesap CRM), LPTracker and HubSpot.
  • Data destinations: Google Sheets, Notion, MySQL and PostgreSQL databases.
  • Messaging and analytics: Telegram notifications for new applications, Google Analytics 4 and a Reostat connection.
  • Payments: a module that accepts online payments through forms created on the service.
  • Webhooks: data can go "using the POST or GET method to an arbitrary URL" after a form is filled in successfully.

Other modules schedule new meetings and register participants from a form, and let a visitor save a form and finish it later. The company also says Form Designer can integrate with "thousands of other applications." The integrations are phrased as automatic transfers: a lead from a form lands in the CRM, a new subscriber lands in the mailing list.

The evidence from the pages

Trial and plans. The home page offers "Try it free" with no credit card required and a 7-day trial period. The template catalog marks each template with a tier, Free, Gold or Platinum: an event evaluation survey is Free, a hostel cost calculator with a discount and a stretch ceiling quiz are Gold, and a kitchen order quiz that captures the buyer's wishes and offers a discount is Platinum. AI assistant use draws on "your plan's AI credits." The site has a Prices page, and one customer comment says "paid subscription plans are also optional."

Customer comments. The home page carries testimonials. One, signed Dmitry Smirnov, says "Almost unlimited possibilities for customizing forms, design, automatic messages to users and administrators" and adds that the webhook support leaves "basically no restrictions on integration." Another says the writer made many feedback forms with different parameters in a corporate graphic style, with "no programming, no fuss with the settings for sending application messages." A third says "All you need is 5 minutes to master the service and ... the forms are ready!" Another reviewer, writing about a budget organization's website, says the tool lets them "actively interact with visitors to our site for free." A further review comes from the Amore + Fiori Federal Flower Network, which thanks the company for "professional, high-quality and prompt service." These are customer statements published by the company, not measured results.

Support and services. The site has a Contacts & Support page, a knowledge base, a page for developers and a partner page, and a reviewer says the support service helps quickly. For teams short on time, there is a paid-service option: "Order turnkey form development," where the company builds web forms for you. Its prompt reads "No time to figure out all the settings yourself? No problem!"

Who it suits and who it does not

Form Designer suits a small business owner or marketer who needs a working form quickly and wants the data to land in a CRM, a mailing list, Google Sheets or Telegram. The calculator and quiz templates are a good fit for a service business that wants to quote prices or qualify leads on its own site. The turnkey service helps agencies and owners who would rather hand over the first build.

It is a narrower tool than a general app platform. The pages describe forms, calculators and booking, with no database apps, portals or approval flows. The ready-made CRM modules are mostly regional services such as Megplan, FreshOffice and Bpium, alongside MailChimp, HubSpot and Notion; for any other system the webhook module is the route. The 7-day trial is short for evaluating a calculator with complex logic, so it pays to have the form planned before signing up.
